  2. Read and Accept that you understand the Provider Expectations document.
  3. Fill out the Programming Plan document
  4. Fill out the Menu Plan document
  5. Click on the Submit button at the bottom of the menu Plan
  6. Fill out the online Provider Application Form at this link - Edmonton Application
