Dear Families/Providers/Staff:

Accreditation for child care services in Alberta is valid for three years. Each year, on the anniversary date of their accreditation, a child care program submits a report to the accreditation body to demonstrate that they are continuing to maintain and enhance the quality of their services. Before the end of their third year, the child care service undertakes the re-accreditation review and has a site visit that is similar to what was completed for accreditation. We are currently in the process of re-accreditation.

The questionnaire below asks for your feedback on various areas that will be addressed during re-accreditation. Please take a few moments to complete the questionnaire below. Your opinions will play an important role in helping us determine what we are doing well and where we can improve.

To protect your confidentiality, including your name is optional. The results from the surveys will be summarized by our family day home agency and incorporated into our Quality Enhancement Plan.
